There are three different kinds of Forge Prize Box: "Glowing" (3★), "Shining" (4★), and "Radiant" (5★) - these correspond to the ★ level of the gear being forged. For example, you won't get a "Radiant Forge Prize Box" while forging a 3★ item like the Swiftstrike Buckler. Prizes will vary in ★ level and amount but will generally be some of these:

Note: Each box has a 100% chance of containing Sparks of Life, as well as ?★ Fire Crystals and Orbs of Alchemy - ★ level is based on what box is opened (Glowing, Shining, or Radiant). Other prizes are random, similar to promotion prize boxes.
